Five area hospitals and doctors groups are trying out a new care model that is designed to improve the quality of care of Medicare patients. Family Practice Group has been selected to participate in this program known as Medicare Accountable Care Organization (ACO). The ACO program intends to provide you with better care and coordination of care and service. The goal of an ACO is for your doctors to communicate closely with you and other health care providers to deliver high-quality care and meet your unique individual needs and preferences.
What this means for you:
You will receive a letter in the mail informing you that Family Practice Group is participating in the Medicare ACO program.
Your Medicare benefits are NOT changing, and this is not a Medicare Advantage plan or an HMO plan.
Medicare will start sharing information with us about your care after (date), to help us give you the right care, in the right place at the right time.
